Any person desiring to secure a Solicitor’s License shall submit to the Town Clerk a written application on forms provided by the Town, verified under oath, containing the following information:

(1) Applicant’s name, address, telephone number, and the name, address, and telephone number of applicant’s employer (if any);

(2) Personal description of applicant;

(3) A full-face, 2x2” or larger photograph showing a clear likeness of applicant;

(4) Applicant’s driver’s license number (if any), and state of issuance;

(5) Vehicle license number of any motor vehicle to be used in conjunction with solicitations;

(6) Uniform Business Identifier number issued by the Washington State Department of Revenue to applicant or applicant’s employer;

(7) A detailed description of the nature or character of services, property, or goods to be offered by applicant; and

(8) Whenever applicable, a copy of applicant’s Food Handler’s Permit issued by the Whitman County Health Department, or registration certificate of license issued by the Washington State Insurance Commission or Washington State Department of Licensing. (Ord. 211-N, §2, 1993; Ord. 95-N, §4, 1977)
